The reader registration form is completed by patrons using books from the 5th Floor Special Collections of Bizzell Memorial Library. These include the History of Science Collections, the Harry W. Bass Business History Collection, the John and Mary Nichols Rare Books and Special Collections, and the William Bennett Bizzell Bible Collection. Hard copy forms are available for completion on site at the research desk in BL5.
